A person walking on the surface or entering a bin to clear bridged material can break through and be buried faster than any rescue can reach them. It is treated as a confined space entry with specific engulfment controls — never entering above stored material, working from outside where possible, and isolating all reclaim before any entry.

Because it happens frequently, it takes only a moment, and production is stopped while it is being done. That combination creates enormous pressure to skip isolation. Stored energy compounds the problem — a jammed crusher, a loaded conveyor or a bridged feeder can release violently when the obstruction is disturbed, even with the drive stopped.
